Cancellations must be submitted using the Cancellation Form. All changes must be submitted using the Change Form.

Cancellations and change request must be received in writing by the appropriate dates:

  • Cancellations or share decreases by the 8th of the prior month
  • Credit card/payment changes by the 8th of the prior month
  • Share increases by 12:00 NOON on the last business day of the prior month.
